We contract with many insurance plans including Medicare and Medicaid to provide medical care to their members. When you request an appointment, please verify that our providers are on your plan. If so, we will file your insurance claim for you. You will be responsible for your plan's office visit co-pay and any deductible at the time of your appointment.

We know insurance details can sometimes be confusing. If you are in a Point of Service Plan or HMO, prior to your appointment please contact your insurance carrier or primary care physician to determine if a referral is needed. Remember to bring your driver’s license, all applicable insurance cards and any forms with you to your appointment.
